I am the parent or legal guardian of the minor child (under 18) named below, and hereby fully release and discharge El Toro Baptist Church d.b.a. Arbor Christian Fellowship and the El Toro Baptist Church International Mission and their officers, employees, and agents from any and all debts or liabilities arising out of or in connection with my child’s attendance of VBS including but not limited to all debts or liabilities arising out of or in connection with the above-described uses of my child’s name, image, and/or voice, or personally identifiable information shared by my child during participation in VBS held on the premises of ACF. To ETBCIM’s possible recording of my child's name, image, and/or voice during participation in VBS held live and in-person, as well as any personally identifiable information my child may share during such event; and agree to the subsequent posting on YouTube, Facebook, and Instagram of such recording(s).
